Items	Author, Title, Quantity, Price For clothing, please indicate Size and Color.
Shipping	Name, Address (no P.O.Boxes, please), City, State, Country, Zip, Daytime Telephone, Email address U.S. and Canadian customers - Shipping is via UPS Ground delivery and charged at $4.00US for the first book and $1.00US for each additional book. Contact us for rates on Express Delivery. Outside North America - Shipping is $18.00US air parcel post or $5.00US surface parcel post for each book. All international orders are sent insured and return receipt requested, so that we know you have received your order. Contact us for rates on Express Delivery.
Tax	Massachusetts residents, please add 5% state sales tax. California residents, please add appropriate state sales tax. Canadian residents, please add 7% GST, and note that your shipment may be subject to Canada Post's border handling fees & other local taxes upon delivery.
Billing	Billing Address (if different form your shipping address), Card type (Mastercard, Visa, American Express, or Discover), Cardholder Name, Card Number, and Expiration Date
